FACELIFT

Facioplasty (facelift) is an elective cosmetic surgical procedure that can remove excess skin as well as tighten the skin. This procedure can conceal age and restore your youthful appearance in a natural way.

Face lifts are designed to reduce the slackness in the tissues of the face; a face lift tightens or excises muscle, fat and skin, producing a more youthful, rested appearance. Generally speaking, face lift procedures only address the two-thirds of the face, although a surgery can combine both face lift and eyelid surgery or browlift.

For individuals interested in reducing sagging in the facial area, face lifts may be the best way to address the problem. For many Miami facelift patients, the key issues corrected by face lift surgery include sagging jowls, loose neck skin, and fat accumulation under the chin (double chin). Face lifts work in essentially the same manner, although there are some variations in face lift techniques, depending upon the specific individual facial constructions, and the intended outcome of the face lift surgery.

A good candidate for a facelift is someone in their late 30s or older, although age does not determine quality of a facelift procedure. The most important factor is skin elasticity, the skin's ability to stretch and heal.

  • The incisions are made. The incisions are the sites that your skin will be stretched. They are usually found in the hairline, front of the ear and under the chin. There is a natural crease at the ear and face. This is typically where the incision is placed. The incision then continues around in front of the earlobe. At the top of the ear it curves back into the hairline. Some incisions extend into the temple hair above the ear and curves slightly forward. Incisions can also be made underneath the chin.
  • The skin is then "lifted." The skin is carefully cut beneath the fat. The skin is gently pulled at the lip, cheek and midline of neck. This is where the tissue is separated delicately. The thin fascia (connective tissue of the face) is also pulled back to provide deeper support to the facial skin, especially around the jaw line. This is called a "Deep" facelift.
  • The tissue is then stitched behind the ear. Any fat is removed to give a sleek and clean look.
  • Some wrinkles may also be removed by gently smoothing the skin back toward the ears.

BROWLIFT

Many people when considering Cosmetic Surgery look toward the face. Often a brow lift is performed alone or with a facelift to give a more youthful appearance.

The brow can show visible signs of aging. The brow lift can repair wrinkles, scowl lines found between the eyebrows at the nose, falling eyebrows, and crow's feet. Often such signs of aging can be lessened through non-surgical means such as scrubs and peels, fat transplants, or botox injections. However, in some cases a brow lift is the best option.

The ideal location of the brow is at the level of the bony orbital rim in a man and about half inch above that for woman. If the position of your brow is below that you may be a good candidate for a brow lift.

For the Coronal lift the surgeon makes two incisions extending from ear to ear. The skin and muscles will be pulled back without extra skin. Sometimes hair needs to be removed as the skin is reattached. The hairline often hides any trace of the scar but it sometimes can look like hair loss if the scar spreads. Other risks with the Coronal lift include scalp numbness and a raised hairline.

Finally, the Subcutaneous lift has an incision directly under your hairline. The skin is pulled upward and the extra skin is removed. The skin is then reattached at the incision. This procedure avoids the risk of raising the hairline with the coronal lift and also does not lift the hairline.

It is important to consider the risks of a brow lift such as visible scarring and numbness. Furthermore, a brow lift is not effective for crow's feet, extra eyelid skin, or poor skin tone. Consequently, many who are interested in a brow lift consider other non-surgical facial cosmetic procedures for further correction. This gives the entire face a more balanced and natural look.

EYELIDS

Blepharoplasty surgery involves the removal or manipulation of the skin, fat, muscle, and cartilage to give a more youthful appearance to the eyes.

The most common changes in the eyelids is the development of excess skin and the protrusion of fat to create a "puffy" appearance. This aging process is usually hereditary but can be affected by sun or tobacco exposure. The brow position also plays an integral part of the eyelid appearance and sometimes requires attention to achieve the best possible result.

Blepharoplasty can remarkably improve the tired, aged, or angry appearance to the eyes to create a more youthful, refreshed appearance.

Sometimes the lower lids can be addressed through an internal incision so that there is no external sign of an operation.

The main risks of this sugery is bleeding, infection, temporary irritation, and an inability to fully close the eyes, or a lowering of the lid.

RHINOPLASTY

The Best Candidates For Rhinoplasty

Rhinoplasty can enhance your appearance and your self-confidence, but it won't necessarily change your looks to match your ideal, or cause other people to treat you differently. Before you decide to have surgery, think carefully about your expectations and discuss them with your Miami rhinoplasty surgeon.

The best candidates for rhinoplasty are people who are looking for improvement, not perfection, in the way they look. If you're physically healthy, psychologically stable, and realistic in your expectations, you may be a good candidate.

Rhinoplasty can be performed to meet aesthetic goals or for reconstructive purposes-to correct birth defects or breathing problems.

Age may also be a consideration. Many surgeons prefer not to operate on teenagers until after they've completed their growth spurt-around 14 or 15 for girls, a bit later for boys. It's important to consider teenagers' social and emotional adjustment, too, and to make sure it's what they, and not their parents, really want.

All Surgery Carries Some Uncertainty and Risk

When rhinoplasty is performed by a qualified plastic surgeon, complications are infrequent and usually minor. Nevertheless, there is always a possibility of complications, including infection, nosebleed, or a reaction to the anesthesia. You can reduce your risks by closely following your surgeon's instructions both before and after surgery.

After surgery, small burst blood vessels may appear as tiny red spots on the skin's surface; these are usually minor but may be permanent. As for scarring, when rhinoplasty is performed from inside the nose, there is no visible scarring at all; when an "open" technique is used, or when the procedure calls for the narrowing of flared nostrils, the small scars on the base of the nose are usually not visible.

In about one case out of ten, a second procedure may be required-for example, to correct a minor deformity. Such cases are unpredictable and happen even to patients of the most skilled surgeons. The corrective surgery is usually minor.

Be sure to tell your surgeon if you've had any previous nose surgery or an injury to your nose, even if it was many years ago. You should also inform your surgeon if you have any allergies or breathing difficulties; if you're taking any medications, vitamins, or recreational drugs; and if you smoke.

Don't hesitate to ask your doctor any questions you may have, especially those regarding your expectations and concerns about the results.

The Surgery

Rhinoplasty usually takes an hour or two, though complicated procedures may take longer. During surgery the skin of the nose is separated from its supporting framework of bone and cartilage, which is then sculpted to the desired shape. The nature of the sculpting will depend on your problem and your surgeon's preferred technique. Finally, the skin is redraped over the new framework.

Many plastic surgeons perform rhinoplasty from within the nose, making their incision inside the nostrils. Others prefer an "open" procedure, especially in more complicated cases; they make a small incision across the columella, the vertical strip of tissue separating the nostrils.

When the surgery is complete, a splint will be applied to help your nose maintain its new shape. Nasal packs or soft plastic splints also may be placed in your nostrils to stabilize the septum, the dividing wall between the air passages.

After Your Surgery

After surgery-particularly during the first twenty-four hours-your face will feel puffy, your nose may ache, and you may have a dull headache. You can control any discomfort with the pain medication prescribed by your surgeon. Plan on staying in bed with your head elevated (except for going to the bathroom) for the first day.

You'll notice that the swelling and bruising around your eyes will increase at first, reaching a peak after two or three days. Applying cold compresses will reduce this swelling and make you feel a bit better. In any case, you'll feel a lot better than you look. Most of the swelling and bruising should disappear within two weeks or so. (Some subtle swelling-unnoticeable to anyone but you and your surgeon-will remain for several months.)

A little bleeding is common during the first few days following surgery, and you may continue to feel some stuffiness for several weeks. Your surgeon will probably ask you not to blow your nose for a week or so, while the tissues heal.

If you have nasal packing, it will be removed after a few days and you'll feel much more comfortable. By the end of one or, occasionally, two weeks, all dressings, splints, and stitches should be removed.

Getting Back to Normal

Most rhinoplasty patients are up and about within two days, and able to return to school or sedentary work a week or so following surgery. It will be several weeks, however, before you're entirely up to speed.

Your surgeon will give you more specific guidelines for gradually resuming your normal activities. They're likely to include these suggestions: Avoid strenuous activity (jogging, swimming, bending, sexual relations-any activity that increases your blood pressure) for two to three weeks. Avoid hitting or rubbing your nose, or getting it sunburned, for eight weeks. Be gentle when washing your face and hair or using cosmetics.

You can wear contact lenses as soon as you feel like it, but glasses are another story. Once the splint is off, they'll have to be taped to your forehead or propped on your cheeks for another six to seven weeks, until your nose is completely healed.

Your surgeon will schedule frequent follow-up visits in the months after surgery, to check on the progress of your healing. If you have any unusual symptoms between visits, or any questions about what you can and can't do, don't hesitate to call your doctor.

Your New Look

In the days following surgery, when your face is bruised and swollen, it's easy to forget that you will be looking better. In fact, many patients feel depressed for a while after plastic surgery-it's quite normal and understandable.

Rest assured that this stage will pass. Day by day, your nose will begin to look better and your spirits will improve. Within a week or two, you'll no longer look as if you've just had surgery.

Still, healing is a slow and gradual process. Some subtle swelling may be present for months, especially in the tip. The final results of rhinoplasty may not be apparent for a year or more.

In the meantime, you might experience some unexpected reactions from family and friends. They may say they don't see a major difference in your nose. Or they may act resentful, especially if you've changed something they view as a family or ethnic trait. If that happens, try to keep in mind why you decided to have this surgery in the first place. If you've met your goals, then your surgery is a success.

CHIN IMPLANTS

A chin implant can augment the jaw-line to correct a weak or retrusive chin and improve the overall harmony of the face and the neck-chin angle. The surgery is done through a small incision under the chin and is associated with relatively quick healing with minimal bruising The main risk of this procedure is the possibility of infection which may require a temporary removal of the implant.

The risks of implants can be that they are too large. If the implant is too large it can create unnatural angles and shadows on your face. There is also a risk of infection or rejection of the implant.

TCA & PHENOL PEELS

Trichloroacetic Acid (TCA) Peels

Trichloroacetic acid (TCA) is typically used as an intermediate-to-deep peeling agent in concentrations of 10 to 35%. This type of peel is useful for patients with surface wrinkles, superficial blemishes, and pigmentation problems. TCA may be used on the neck as well as other areas of the body such as the hands, forearms, and chest. The treatment is rapid, requiring only thirty minutes from beginning to end. There is some burning upon application, and intravenous sedation may be necessary.

Once the treatment is completed, you can expect to shed the dead, outer layer of skin for approximately three days. Following this, you can expect your skin to be completely healed within ten to fourteen days. At that time you will notice improvement in the appearance of your skin, which will have less wrinkles, blemishes, and pigmentation problems. You will also notice a healthy glow as your face appears radiant and shining.

TCA is a deeper peel than most of the superficial peels discussed in this section. Because it is a deeper treatment, your "downtime" will be approximately one week. During this time of peeling and healing, you will be required to keep your facial skin moist with A&D Ointment or Vaseline at all times. After you are healed, sunscreen use is very important because you are initially much more sensitive to the harmful ultraviolet radiation from the sun.

Phenol Peels

A deep chemical skin peel, or phenol peel, is the strongest of chemical peels, and is reserved for individuals with deep wrinkles from sun exposure or is used to treat skin wrinkling around the lips and chin area. The procedure diminishes imperfections in sun-damaged, unevenly pigmented, or coarsely wrinkled facial areas by peeling away the skin's top layers.

Technique: A full-face deep chemical peel takes 1 to 2 hours to perform. A more limited procedure (such as treatment of wrinkling above the lip) will generally take less than a half-hour. A solution is applied to the area to be treated (avoiding the eyes, brows and lips). There is a slight burning sensation, but it is minimal since the solution also acts as an anesthetic. After the peel solution has worked on the skin, it is neutralized with water. Approximately one hour later, a thick coating of petroleum jelly is layered over the patient's face, covering the protective crust which develops rapidly over the area. This stays in place for 1 to 2 days. In an alternative technique, the patient's face is covered by a "mask," composed of strips of adhesive tape, with openings for the eyes and mouth (this is particularly effective in cases of severe wrinkling).

Some patients experience discomfort after a deep chemical peel, but this can be controlled with medication. A few days after the procedure, new skin with a bright pink color akin to sunburn will emerge; the pinkness will fade within a few days. Post-operative puffiness will also subside in a few days, but the skin will remain sensitive. Patients should avoid exposure to sunlight and continue to use sun block.

Benefits:

  • Effects of a phenol chemical peel are long lasting, and in some cases are still readily apparent up to 20 years following the procedure.
  • Improvements in the patient's skin can be quite dramatic. Normal work schedule and other activities can be resumed after 1 to 2 weeks.
  • Variants in the phenol peel formula can create a milder solution for broader use.

Other Considerations:

  • Possible postoperative complications can include scarring, infection or abnormal pigmentation. Tends to have a bleaching effect, and patient may need to wear make-up to match treated and untreated areas.
  • Cannot be used on the neck or other parts of the patient's body.
  • Not as effective in treating individuals with dark, oily complexions.
  • Some facial skin disorders do not respond to chemical peeling.
  • Skin pores may appear larger, and the skin may not tan properly.
  • Can activate latent cold sore infections.
  • All forms of deep skin peels include the risk of delayed healing and scarring.
